Case Study 1:
The patient was concerned that her nose was big and wide. Her preoperative photo shows her frontal view to have some increased width at the bridge and tip ptosis. Her base view shows the ptosis and some risk of nostril collapse on narrowing. Her lateral and oblique views show some profile prominence, tip ptosis and under projection and some alar columella disproportion and retraction.
I felt this could be done through a closed approach since I didn’t think that the profile needed a major reduction, that the tip could be rotated and the alar columellar relationship improved with good control without excess sutures or grafting.
Her post operative photos show her frontal view with good narrowing of her bridge, tip elevation without nostril show and excess pinching, improvement in the alar columellar relationship. Base view shows nice triangularity to the tip with good support of the alar sidewalls. Lateral and oblique views show correction of the dorsal profile prominence and alar columella relationship.
