The term "open" rhinoplasty is used when an incision is made on the external columella skin in addition to internal incisions. Some surgeons prefer this approach others the endonasal approach. Some do both depending on what is needed for the individual patient. There is no one right way to do a rhinoplasty. Getting the result is what counts, not the steps the doctor takes to get there.
